Important: If you are enrolled in a MA Plan, PACE Plan, or Medicare Part D Plan, enrollment in Tufts Medicare Preferred will result in disenrollment from your current plan.

1) By Phone:

1-800-521-3062 (TDD 1-888-899-8977) Our representatitves are happy to take your application over the phone, just call Monday - Friday, 8:30 a.m. - 5:00 p.m. You can also enroll by calling 1-800-MEDICARE.

2) Online:

Click here to enroll online through Tufts Health Plan Medicare Preferred Online Enrollment Center.

3) By Mail:

Download and print an enrollment form from the Quick Links box above. Once completed, mail the enrollment form to: Tufts Medicare Preferred 705 Mt. Auburn Street Watertown, MA 02472-1508

4) By FAX:

Request an Enrollment Package:
At any time you may request an enrollment package by calling 1-800-528-3818 (TDD 1-888-899-8977) Monday - Friday 8:30 a.m. - 5:00p.m. Our representatives will be happy to mail you an enrollment kit with more information and an enrollment application.

You may also complete and submit a Request for Plan Information form in order to receive an enrollment kit in the mail.
